New Patients - If you are a new patient, please book an Initial Assessment below. Once your first appointment (New Chiropractic or Acupuncture Patient) is booked, you will receive an email confirmation which contains a link to the intake form. This intake form must be filled out completely prior to your first appointment.

Credit Card on File - A credit card on file is required to book any appointment online. This card will not be charged unless requested by the patient or for late cancellation, late reschedule, or no show fees. Consultations are always complimentary. However, a fee of $15 will be charged if a patient does not “show up” for the consultation. HSA/FSA and all major credit cards are accepted for payment at time of service.

Insurance - ANSH Chiropractic & Wellness now accepts certain insurance plans including UHC, Anthem, Blue Cross Blue Shield plans. If you do not have insurance, superbills (form needed to submit the out of network claim to insurance) are provided upon request.

Good Faith Estimate - Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You will be provided with a Good Faith Estimate at the end of your initial visit. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
